Sports Physicals $50
Monday though Fridays, 10am to 6pm
Weekends and Holidays, 10am to 3pm
Physicals are available at all locations, If the clinics are especially busy or a provider has high acuity patients that require more attention, patients requesting physicals may be asked to wait or return at a later time at the provider’s discretion.
Sports physicals are available for children in grades 1 through 12.
If your town has its own form, bring it with you.

School Physicals $75
School physicals are available for children entering grades 1through 12. This cost includes an H&H if required.
School physicals are available for children entering or in grades 1 through 12.
Patients must bring a full copy of the child’s vaccination records, or a copy of their notarized religious or medical exemption form. If the child is not up to date and requires vaccines, they must have them done and come back with documentation of such before the physical can be performed. Patients who are missing vaccines may be referred to their pediatrician, family doctor or to the local health department.
If a minor is brought in for a physical by someone other than their parent, identity of the adult must be verified along with verbal or written permission to treat
from the child’s parent. If the child is brought in by a legal guardian or foster parent, copies of guardianship or temporary custody papers must be provided. Exchange or boarding students who come in with a school staff member or host parent, please provide a copy of the consent to obtain medical treatment signed by your parent.
